Cameron cancels a vacation with Chase, saying she wanted to make sure House takes the case of an environmental activist (Tim Rock) who collapsed at a rally. House first suspects Cameron is trying to take Kutner’s job, and gets her to do all the initial tests and questions, while Chase suspects Cameron may still be in love with House. Wilson, meanwhile, begins a healthy diet, causing House to question his motives.

A young priest, Fr. Daniel Bresson, runs a homeless shelter. After he gives a man a sweater to keep warm, he breaks out some liquor and begins to drink at a dangerous rate. He hears a knock at his door and opens it, horrified to find an apparition of a bleeding Jesus Christ hovering a foot off the ground. Fr. Bresson passes out and is taken to Princeton-Plainsboro. House takes the case from Cameron, who tells him there is nothing to diagnose, as Fr. Bresson simply drank too much bourbon. House seems to realize this, but takes the case as a diversion for Kutner and Taub while he confronts Foreman and Thirteen about their relationship. While the team test Bresson’s EEG for epilepsy, House calls Foreman and Thirteen in. He gives them the option to “split or quit,” as he feels their relationship is clouding their judgment as doctors and diagnosticians. He tells them they have until the end of the case to decide.

House and the team take on the case of a woman who collapsed in the middle of a cooking class. When they learn that the patient gave up her career as a highly renowned cancer researcher in order to pursue her own personal happiness, the members of the team question their own happiness (or lack thereof). Meanwhile, Thirteen begins to suffer serious and life-threatening reactions to her experimental Huntington’s Disease clinical drug trial, becoming blind due to a brain tumor. Cuddy retaliates against House, giving him a taste of his own medicine.

A troubled teen, Natalie, is singing at her school’s Christmas program when she suddenly becomes disoriented and vomits. She is brought to Princeton-Plainsboro Teaching Hospital, where the team discovers her liver is failing for an unknown reason. In House’s office, they are exchanging Christmas gifts when House receives a mysterious gift wrapped in green wrapping paper. Kutner reads the note aloud, as it says “Greg- made me think of you.” The present turns out to be Dr. Joseph Bell’s Manual of The Operations of Surgery. House, annoyed, throws it in the trash. Afterwards, House conducts a differential diagnosis, with the team entertaining ideas such as alcohol abuse and pill-popping. None of the symptoms fit with the entire picture. When Chase and Kutner question Natalie’s classmates, it turns out that they gave her magic mushrooms, prompting the doctors to think that this whole episode was induced by others.

When a fitness guru filming a promo for her new DVD becomes unconscious and falls down, breaking her ankle, she is sent to Princeton-Plainsboro Teaching Hospital, under the care of House and his diagnostics team. Meanwhile, Foreman begins his clinical trials on Huntington’s disease, with Thirteen partaking in the drug regimen, although she is less than thrilled about it. Over the course of the patient’s treatment, the patient develops loss of muscular and nerve control throughout her body and hallucinations. When she admits to having gastric bypass surgery (prior, she was overweight), Taub accuses her of being a hypocrite, since she did not go through the sort of routine exercises and workouts her fans are going through to lose weight. Frustrated with the lack of an actual diagnosis and endless futile testing, the patient asks for chocolate cake to mope over her impending demise, but when she becomes healthier after eating the treat, House deduces the patient has a lack of an enzyme in her body, known as hereditary coproporphyria, causing her symptoms.

Sixteen year old Sophia is an emancipated minor working at a factory when she suddenly clutches her chest and coughs up blood. Meanwhile, House comes into Princeton-Plainsboro Teaching Hospital and Wilson claims he did not ask Cuddy out; although Wilson does not comment, House suspects something is up. The team takes on the case and Foreman suggests Sophia’s collapse is due to pregnancy, but Kutner does not believe it, nor the notion that she is taking drugs, as Thirteen suggests. House orders an echo, but Foreman asks to be let go temporarily to do clinical trials. House refuses, claiming he might need him for the current case. Taub and Thirteen go to Sophia’s apartment and discover a bong, suggesting drug use, but Kutner believes Sophia needs steroids for vasculitis. Unfortunately, House tells Foreman to put her on beta-blockers, yet Foreman informs the rest of the team to take care of it and leaves.
